To apply for membership to Mansfield Health and Fitness Centre, you can either :

fill out a form on line or

click on the form shown.

This will give you a pdf version for you to print out and fill in.

• You can either e mail the form back to us on steve@mansfieldhealthandfitness.com.au
• post it to 13 Highett St Mansfield Vic 3723 or
• bring it in to the gym.
